About the Healthcare Provider
A leading multi-specialty healthcare network in the northeastern United States, serving a diverse population with world-class medical services across cardiology, oncology, radiology, primary care, hospice, and rehabilitation.
The organization operates a comprehensive continuum of care, spanning inpatient, outpatient, and specialty services, supported by over 4,000 skilled healthcare professionals. Beyond its acute care hospital, the network includes specialty centers, advanced diagnostic facilities, and community-based clinics.
Recognized for clinical excellence, innovation, and its patient-first philosophy, the provider is deeply committed to delivering compassionate, technologically advanced healthcare tailored to the needs of its growing regional population.
Driving Agility, Coordination, and Excellence in Patient Care
As the provider expanded its clinical footprint, its on-premise DB2 Z-Series legacy platform became a major constraint- unable to keep pace with the demands of integrated, patient-first care. Disconnected systems, manual processes, and rigid infrastructure slowed down operations and limited scalability.
Key operational bottlenecks included:
- Patients re-entering information multiple times across sites due to fragmented registration systems.
- Nurses manually chasing lab results and pharmacy updates, delaying treatment initiation.
- Physicians toggling between siloed dashboards to compile patient histories, diagnostics, and care plans.
- Scheduling conflicts and administrative overlaps leading to appointment delays and resource wastage.
- Discharge planning slowed by lack of real-time bed and resource availability visibility.
- Insurance verification, pre-authorization, and referrals handled manually, increasing cycle times.
- Staff fatigue from administrative overload, diverting time from direct patient care.
- Inconsistent patient experiences across inpatient, outpatient, and specialty care settings.
Bottom line: Without a unified, agile digital foundation, care teams were constrained in their ability to deliver timely, coordinated, and compassionate care. Operational inefficiencies rippled into patient experiences- delaying diagnostics, extending wait times, and fragmenting care journeys at a time when speed, safety, and continuity mattered most.
Building a Patient-Centric Foundation for Transformation
Before launching modernization, TVS Next conducted a deep-dive discovery phase, collaborating with clinical, administrative, and IT teams to uncover operational gaps and digital readiness.
The focus was clear: deliver measurable improvements, protect ongoing care operations, and lay a scalable foundation for transformation across five incremental milestones.
Key steps in our approach included:
- Cross-functional workshops to identify legacy bottlenecks, system gaps, and workflow inefficiencies.
- On-site workflow observations across emergency intake, lab coordination, and admissions to map operational friction points.
- Data architecture reviews to diagnose fragmentation across patient registration, clinical records, pharmacy, and billing.
- Prioritization of transformation milestones, sequenced to deliver early value while building long-term capability.
- Flexible adoption of the Build-Operate-Transfer (BOT) model, ensuring gradual, confident ownership transitions.
- Continuous feedback loops between IT, operations, and care delivery teams to refine execution in real-time.
This foundation enabled each milestone to drive faster, safer, and more coordinated care delivery while modernizing the provider’s digital ecosystem for long-term growth.
Transformation – 5 Incremental Milestones
Milestone 1
Legacy Stabilization & Foundation Setup
What we did: We began by supporting and decoding the DB2 legacy stack, replacing Mirth with InterSystems Ensemble for HL7 handling, and setting up a secure environment. A scalable microservices architecture was laid on Azure, with the Emergency Department selected as the first rollout zone for live deployment.
Impact: Reduced disruptions for patients, stable tools for staff, and reliable partner handoffs.
Milestone 2
Clinical Integration & Platform Engineering
What we did: This phase established a shared data estate and integrated labs, pharmacies, and agencies using HL7. Emergency workflows were powered by real-time updates. Platform engineering enabled consistent APIs, identity layers, and observability for downstream systems.
Impact: Faster diagnostics, unified records, and better clinical decisions at the point of care.
Milestone 3
Workforce Enablement & Patient 360
What we did: We introduced Angular-based apps for enterprise enrollment, shift scheduling, and queue management. A Patient 360 view unified historical and live data. Practice management tools streamlined day-to-day operations across clinics and specialties.
Impact: Shorter wait times, empowered staff, and seamless transitions for patients.
Milestone 4
Compliance, ICU Workflow & Observability
What we did: We implemented HIPAA/HL7-compliant pipelines with audit trails and automated rollback. Real-time ICU dashboards improved handoffs. Referrals, HR records, and hospice integration workflows were digitized to improve transparency and safety.
Impact: Safer transitions, audit readiness, and stronger staff communication.
Milestone 5
Ecosystem Activation & Ownership Transfer
What we did: Rolled out to 100+ clinics with secure APIs and partner portals. Embedded clinical decision support in cardiology, oncology, and radiology workflows. Revenue cycle automation shortened billing cycles. Internal teams received playbooks, simulators, and training for full handoff.
Impact: Cohesive care experience, improved clinical decisions, and operational independence.
Transition of BOT from Operate to Transfer
Following the stabilization and optimization phases under the Operate phase of BOT, we executed a structured and transparent Transfer process. Key elements included
- Operational Readiness Assessments: Conducted a series of readiness workshops to evaluate the maturity of internal teams across platform management, clinical workflow adoption, and incident
- Knowledge Transfer Acceleration: Established a phased transition plan involving parallel run periods where TVS Next teams and internal stakeholders co-managed operations, progressively shifting
- Playbooks and Simulations: Delivered detailed operational playbooks, conducted simulator-based dry-runs, and provided scenario-based troubleshooting guides to empower internal teams.
- Certification and Sign-off: Built a competency certification framework allowing internal leads to demonstrate system mastery before formal handover.
- Transition Governance: Set up a transition governance board with representation from IT, clinical leadership, and operations to oversee a risk-managed handover.
- Post-Transfer Support Window: Offered a 90-day hyper-care window to ensure seamless stabilization, knowledge reinforcement, and confidence building among the client’s teams.
This ensured that the transition from Operate to Transfer was smooth, strategic, and sustainable- enabling the provider to fully own, operate, and scale the modernized ecosystem independently.
